Terms and Conditions
1. Introduction
These Terms and Conditions set out the basis upon which medical services are provided by ICM Private GP, a private medical practice based in Cardiff.
By booking an appointment or receiving treatment at the Clinic, you agree to these Terms and Conditions.
2. Scope of Services
The Clinic provides private primary care medical services, including but not limited to:
· General practice consultations (face-to-face and remote)
· Preventative healthcare and health screening
· Management of acute and chronic conditions
· Menopause care
· Dermatological/skin condition assessment and treatment
· Weight management services
· Hair and follicular health consultations
· Diagnostic investigations (where available or arranged externally)
· Prescriptions (private prescriptions unless otherwise stated)
The Clinic provides private medical services within the recognised professional scope of General Practice. All clinical services are delivered in accordance with:

· The professional standards and ethical duties set by the General Medical Council (GMC);
· Applicable Welsh healthcare legislation and regulatory requirements;
· The requirements of Healthcare Inspectorate Wales (HIW), where applicable.
The Clinic does not provide secondary or tertiary care services and does not hold itself out as a specialist service beyond the remit of general practice.
Clinical assessment, diagnosis and management are undertaken based on the information made available by the patient at the time of consultation. Patients are responsible for providing accurate, complete and up-to-date information regarding their medical history, medications and relevant circumstances.
Where, in the reasonable clinical judgment of the practitioner, a patient’s condition:
· Falls outside the scope of general practice;
· Requires specialist investigation, intervention or multidisciplinary input;
· Requires emergency or urgent secondary care assessment; or
· Cannot be managed safely within the Clinic setting,
the practitioner will advise appropriate onward referral without delay.
Referral may include:
· Direction to the patient’s NHS General Practitioner;
· Referral letter to the patient’s NHS General Practitioner;
· Recommendation of an appropriately qualified private specialist; or specialist opinion from specialist colleagues;
· Advice to attend urgent or emergency care services where clinically indicated.
The act of providing referral advice or issuing a referral letter will not incur an additional professional fee. However, the Clinic accepts no responsibility for the availability, timeliness, costs, clinical decisions, or outcomes of care provided by external healthcare providers. Any fees charged by third-party providers remain the sole responsibility of the patient.
The Clinic reserves the right to decline to provide or to discontinue treatment where it determines that:
· The requested treatment is not clinically appropriate;
· The matter exceeds the safe or lawful scope of general practice;
· Adequate clinical information is not available;
· The patient-doctor relationship is deemed unsuitable for continuation of care, by the doctor, or
· Continuing care would not be in the patient’s best interests.
In all cases, the Clinic’s overriding duty is to ensure patient safety, appropriate clinical governance, and compliance with regulatory standards.

Where clinically indicated, blood tests and other diagnostic investigations may be arranged as part of the patient’s assessment and management plan. The Clinic does not operate an on-site laboratory. All laboratory testing and diagnostic investigations are outsourced to appropriately accredited and competent external providers.
Such providers will be suitably qualified, regulated and compliant with applicable UK legislation and quality standards, including accreditation by the United Kingdom Accreditation Service (UKAS) where required, and adherence to relevant clinical governance and quality assurance frameworks.
The Clinic undertakes due diligence in selecting third-party laboratories and diagnostic providers to ensure they meet recognised professional, safety and regulatory standards. However, the Clinic is not responsible for operational delays, laboratory processing times, or errors attributable to external providers.
Patients will be informed where investigations are performed by third-party organisations, and any associated costs will be clearly communicated. Fees charged by external laboratories or diagnostic providers remain the responsibility of the patient unless otherwise expressly stated.
All results received by the Clinic will be reviewed by the responsible clinician and communicated to the patient in a timely and clinically appropriate manner.

The Clinic is not responsible for:
· Delays caused by third-party providers
· Outcomes where treatment advice is not followed

· Data protection legislation (including UK GDPR)
· Relevant healthcare regulatory requirements
· Healthcare Inspectorate Wales (HIW), where applicable
10. Confidentiality and Data Protection

If the patient shares information that might put others at risk, doctor-patient confidentiality might be breached following a discussion with 2 professional peers as well as with the adequate medical defence organisation, with knowledge of the patient.
Information relating to care provided at the Clinic will be recorded accurately and securely within the patient’s medical record.
In order to promote continuity, patient safety and coordinated care, relevant information regarding consultations, diagnoses, treatment plans, prescribed medication, and investigation results will ordinarily be shared with the patient’s registered NHS General Practitioner, unless requested by the patient otherwise.
This information sharing:
· Is undertaken in the patient’s best interests;
· Supports safe prescribing and avoidance of duplication or contraindication;
· Ensures appropriate ongoing care within NHS services where required.
Where practicable, patient consent for information sharing will be obtained. However, information may be shared without explicit consent where there is a lawful basis to do so, including where it is necessary for direct care, safeguarding, public protection, or where required by law.
Patients who do not wish information to be shared with their NHS GP should inform the Clinic. The clinician will explain any potential risks to patient safety or continuity of care that may arise from restricting such information sharing. The Clinic reserves the right to decline or limit treatment where withholding information would pose a significant clinical risk.

Any concerns or complaints should be raised in writing to ICM Private GP, via email provided or by post. Complaints will be handled with understanding and cooperation.
